Four physicians staff the Plastic Surgery & Aesthetics Medical Center at Chongqing West District Hospital. Dr. Tongchun Mao is the responsible physician for this site and assesses its lower-lid cases; the Center Director sets the standards the practice is held to, and two team physicians carry the perioperative and follow-up care that shapes how a lid settles.

Tongchun Mao

Tongchun Mao (毛彤春)

Deputy Director & Head of Aesthetic Surgery · Vice Chief Physician, Licensed Aesthetic Attending · Lead Physician of This Site

Lower-lid assessment · eyelid and periorbital surgery · facial contouring

Dr. Tongchun Mao is an aesthetic surgeon at Chongqing West District Hospital, where he is Deputy Director and Head of Aesthetic Surgery in its Plastic Surgery & Aesthetics Medical Center and the responsible physician for this site. He personally performs more than 700 operations a year, within hospital standards of assessment, anaesthesia and follow-up.

His approach to the lower lid is to name the finding before naming an operation. That means examining lid tone and position first, asking about dryness, watering, thyroid history and fluid retention, and deciding whether the fullness is a fat pad, a bulging muscle, loose skin or a hollow read as a bag. Where the honest answer is that the complaint is fluid, that it comes from the brow, or that the lid is too weak for the operation the patient came in asking for, that answer is given at the assessment rather than discovered afterwards. Where an operation is right, the plan states what will be left alone as clearly as what will be changed, and the result is judged at six months rather than at six weeks.

Clinical focus

  • Assessment of lid tone, position and the tear film before any operation is planned
  • Matching the operation to the dominant finding rather than to the label
  • Fat repositioning where a bag sits above a hollow, and restraint where it does not
  • Staged planning when more than one finding needs to be judged separately
  • Honest referral to another specialty where the cause is medical rather than structural

The operation that is declined

A lower-lid practice is defined as much by the operations it does not do as by the ones it does. Where the lid tone is poor, the eye is already dry, or the complaint is not structural, the plan changes or ends. Patients are told that plainly at the assessment, because a lid that has been over-operated is harder to help than one that has not been operated on at all.

Find out which operation is being offered, and whether you need one

Arrange an assessment of the lower lids. We examine lid tone and position, the tear film, and whether what you see is fat, fluid or skin; we name which of the four operations answers it and which is being left alone; and we say plainly that fat taken does not come back, and that the honest answer is sometimes not an operation.
